The Wellness Center Nurse Navigator at The Clare arranges health-related programming, coordinates the visits of a variety of medical providers, facilitates resident transitions from hospital stays back home and arranges for a visiting lab technician to complete lab orders per doctor requests. Whether a resident has a planned surgery or makes an unexpected trip to the emergency room, the Wellness Center is there to check in. They will consult follow-up orders from doctors and help to arrange for therapy services or home healthcare as needed.
